Chin augmentation from malalignment, serious injury or birth defects can be done by using chin implants (facial implants) or with a sliding genioplasty. Dr. Elias refers to this sliding genioplasty as an osseous genioplasty. Osseous genioplasty offers clients that have receding chin to cut a portion of the mandible, making up the chin bone, and repositioning it forward to help with facial symmetry. Mentoplasty refers to facial implant placement to enhance the structure of the chin or angle. Depending on the type of condition and severity of the chin position, Dr. Elias of Southern California Center for Surgical Arts will recommend one or the other.

There are a few reasons clients choose to modify the chin:

Retraction

In contrast to receding chin, some clients also experience the opposite positioning of a protruding chin. The mandible may be overextended and can cause this forward jutting of the chin that can cause discomfort or confidence issues. In this case, Dr. Elias will offer some options to correct this protrusion of the bone by cutting down the mandible and readjusting the positioning. Some skin excision may be needed if enough of the bone is taken out. Extraction of the bone in the mandible will usually be taken from the portion behind the teeth. Dr. Elias will pull back the bone to reduce the projection of the chin; should you need dental work to enhance the overall look of the chin and teeth combination, Dr. Elias can provide orthodontic treatment as well.

Advancement or Projection

This is the basic treatment request for genioplasty that requires forward movement of a receding jawline. As expressed above, Dr. Elias can cut a small portion of the mandible bone forming the chin and slide it forward. This sliding of the bone in a forward position will allow for a more defined chin and can be set in place by a small metal piece that is screwed into the bone. Depending on the amount of forward movement required to evenly place the chin in proportion to other facial features, the movement increments begin at six millimeters and can extend to twelve millimeters. The second option is for placement of a facial implant to help with slight shape modification of the chin if there isn’t a significant length needed to move forward or backward; but this implant technique is mostly used in adjustment of the chin.

Adjustment

Chin adjustment can refer to the overall movement in a forward or backward projection to help reshape the delicate lines of the face. With reference to the nose, ears, mouth and other features, chin adjustment or reshaping can help our clients to achieve their desired result. This type of chin modification requires the use of facial implants in the chin to add more definition or volume to the area. Some clients want to soften the lines of the chin, if the chin is pointed or too round. The chin can be reshaped and adjusted with these safe and easily placed implants.

Common congenital conditions that will eventually require genioplasty include:

  • Retrognathia a specific condition acquired at birth that refers to the receding positioning of the chin. This could also refer to the maxilla or mandible.
  • Microgenia or hypoplasia is a condition that is most treated in young children. It refers to the abnormal sizing of the jaw.

These conditions causing micrognathia are fairly common in children at birth:

  • Marfan’s syndrome: improper growth of connective tissue and the support they offer for the bone structure or organ formation. This condition is genetic and can be inherited, but it is the cause of insufficient protein growth.
  • Crying cat syndrome: This condition is caused by malformed bone structure in the jaw and disproportion in the larynx. A crying cat sound is produced by children that have this condition and can be shrill—a distinct sound.
  • Achondrogenesis: A rare condition, achondrogenesis causes softness in the bones. These children break bones all the time and are malformed. Weak or small chin results from this inability to grow correctly; the condition is genetic.

Recovery

Chin augmentation or genioplasty recovery time can vary anywhere from 10-14 days time. Dr. Elias will go over all of your post-operative care with you before and after surgery. Should you require pain medication, he will prescribe some for duration of this recovery period.
